Corrective Actions: E-ONE will notify owners by mail and advise you to take your fire truck to a dealership to update the vehicle multiplex software.

  Issue: On certain fire trucks, an engine crossmember may not have been manufactured properly. As a result, the crossmember could break and the engine cooling fan could become damaged. If this happens, the engine could overheat and cause a loss of power to the wheels. This could also result in the truck not being able to pump water. Safety Risk: A loss of power to the wheels could increase the risk of a crash. Additionally, a fire pump that doesn't work could cause a lack of water pressure for fighting fires. This could delay emergency response and increase the risk of injury. Corrective Actions: E-ONE will notify owners by mail and advise you to take your truck to a dealership to add braces to strengthen the crossmember. If the crossmember is damaged, it will be replaced.

Safety Risk: A change in steering effort could increase the risk of a crash. Corrective Actions: E-ONE will notify owners by mail and advise you to take your truck to a dealership to inspect the steering assist bracket and, if necessary, replace it.

Corrective Actions: E-ONE will notify owners by mail and advise you to contact Weldon to update the Weldon power distribution unit software.

Note: This recall is for fire trucks equipped with certain Square D QO plug-on neutral load centers. Safety Risk: An overheated load center could create the risk of a fire. Corrective Actions: E-ONE will notify owners by mail and instruct you to contact Schneider Electric to arrange for an inspection. The load center will be repaired or replaced, as necessary.

  Issue: On one fire truck, there could be a problem with circuit board for the electrical multiplex module (Weldon V-Mux Hercules HC 1.5 node). This could cause some vehicle systems not to work properly, including the headlamps and taillamps. Safety Risk: A loss of lighting could cause reduced visibility and increase the risk of a crash. Corrective Actions: E-One will contact the owner and provide a replacement electrical multiplex module and instructions for how to install it.

  Issue: On a small number of fire trucks, there may be a defect in the rear axle drive pinion that could cause it to break. If this happens, the air brake line could be damaged and cause the parking brakes to apply suddenly. Note: This recall only affects vehicles equipped with certain 180-series Meritor axles. Safety Risk: A parking brake that self-applies while driving could increase the risk of a crash. Additionally, if a following driver does not recognize when a vehicle ahead is braking, there could also be an increased risk of a crash. Corrective Actions: E-ONE will notify the owners by mail and arrange an on-site axle pinion inspection. The differential carrier housing assembly will be replaced, if necessary.

  Issue: On certain vehicles, the fuel rail assembly could develop a leak. Note: This recall only affects vehicles equipped with a Cummins L9 or ISX12 diesel engine. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: Cummins will notify owners by mail and instruct you to take your vehicle to a Cummins authorized service center to inspect the fuel rail. If damage is found, the fuel rail and fuel lines will be replaced. If no damage is found, the fuel rail will be reassembled with new crush washers using a special tool.

If this happens, you may have difficulty steering or there could be a loss of steering control. Safety Risk: A loss of steering control could create a risk of a crash. Corrective Actions: E-ONE will notify owners by mail and instruct you contact E-ONE customer service. E-ONE will contact Reyco Granning, who will make arrangements with you to replace the steering gear mounting bolts.

  Issue: On certain vehicles equipped with a Weldon Vista IV control panel, an electrical problem could cause the screen to not work properly. As a result, the operator may not be able to control or access certain vehicle functions. Depending on the vehicle configuration, this can affect the emergency warning lights, patient care devices and/or cause the rearview camera image not to display. Safety Risk: Emergency personnel that do not have immediate access to vehicle controls could create a delay in emergency response. Additionally, a rearview camera image that does not display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: E-ONE will notify owners by mail and instruct you to take a photo of the Vista display screen(s) showing the decal with the serial number. E-ONE will verify if each unit is affected and provide a new display, as necessary.

  Issue: On one vehicle, the u-bolts that attach the axle to the suspension may not be properly tightened. As a result, the axle locating dowels could break. Corrective Actions: E-ONE will notify the owner by mail and instruct them to take the vehicle to a dealer to inspect, and if necessary, install a new lower shock mount bracket and new axle u-bolts.

Corrective Actions: E-One will notify owners by mail and instruct them to take their vehicle to a service center to install a relay and diode kit.

A loose shock absorber may contact and damage the steering gear housing, resulting in a steering fluid leak and loss of power steering assist. Safety Risk: A loss of power steering assist could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a service center to inspect and repair the front shock absorber upper mounts.

Correction: Dealers will replace the IOTA switch.

Correction: Dealers or certified technicians will inspect and replace the retaining ring(s) as necessary.

As a result, the filter head may flex under pressure, allowing the sealing O-ring to be pushed out, resulting in a fluid leak. This could cause reduced aerial/ladder performance, increased power requirements and could potentially result in damage to other hydraulically driven components. This could increase the risk of injury/damage to property. Correction: Dealers will inspect the filter head and replace if necessary.

  On certain fire trucks, the components that make up the 6" water intake strainer may have been improperly soldered during the manufacturing process. This could lead to degradation in pump performance which could reduce firefighting effectiveness, and potentially increase the risk of injury. Correction: Hale Products recommends an inspection be performed by pulling caps and any suction adapters off of the intake and check the strainer for faulty soldering. If the strainer is found to be missing solder at the joint, Hale Products will send a replacement strainer directly to affected owners.

  On certain fire trucks, specific operating conditions (such as tight, successive, highly banked curves in opposite directions), could trigger unintended Electronic Stability Control (ESC) system intervention. As a result, the ESC may unnecessarily apply one of the front brakes in order to correct the perceived oversteer condition. This may cause the vehicle to deviate from the intended path, thereby increasing the risk of a crash causing property damage and/or personal injury. Correction: Dealers will replace the ESC module.

  On certain fire trucks equipped with a Cummins engine, the V-band clamps used to connect the inlet and outlet cone sections to the Diesel Oxidation Catalyst (DOC) / Diesel Particulate Filter (DPF) include a T-bolt which can fracture and fail when subjected to stress or load. A fracture or failure of the T-bolt may cause the clamp to loosen and the inlet or outlet sections attached to the DOC/DPF housing to disconnect. Should this occur, hot exhaust gases can vent prior to exiting the tailpipe, creating a risk of combustion or damage to adjacent materials, which could result in a vehicle fire. If both clamps fail, the DOC/DPF housing could detach completely, separate from the vehicle, and strike another vehicle, stationary object, or bystander. These issues could result in property damage and/or personal injury. Correction: Cummins will install updated V-band clamps.

  On certain fire trucks, the door latch fasteners may not have received adequate tightening torque during the manufacturing process. As a result, the outer door handle may rotate out of position, allowing a retention spring to dislocate, which could cause the door to open during a vehicle crash. Should this occur, unbelted occupants could fall out of the vehicle and sustain personal injury. Correction: E-One service centers will replace the door latch assemblies.
