    Issue: On certain fire trucks, a software problem could cause the drive pump transfer case to disengage. If this happens, there could be a sudden loss of power to the wheels. Safety Risk: A sudden loss of power to the wheels could increase the risk of a crash. This could also delay emergency response and increase the risk of injury. Corrective Actions: E-ONE will notify owners by mail and advise you to take your fire truck to a dealership to update the vehicle multiplex software.

    Issue: On certain fire trucks, an engine crossmember may not have been manufactured properly. As a result, the crossmember could break and the engine cooling fan could become damaged. If this happens, the engine could overheat and cause a loss of power to the wheels. This could also result in the truck not being able to pump water. Safety Risk: A loss of power to the wheels could increase the risk of a crash. Additionally, a fire pump that doesn't work could cause a lack of water pressure for fighting fires. This could delay emergency response and increase the risk of injury. Corrective Actions: E-ONE will notify owners by mail and advise you to take your truck to a dealership to add braces to strengthen the crossmember. If the crossmember is damaged, it will be replaced.

As a result, this could cause a reduction of steering assist and/or the steering may make clunking noises. This can cause a change in steering feel and an increase in steering effort. Safety Risk: A change in steering effort could increase the risk of a crash. Corrective Actions: E-ONE will notify owners by mail and advise you to take your truck to a dealership to inspect the steering assist bracket and, if necessary, replace it.

As a result, the unit and/or wiring could overheat and melt. Safety Risk: A power distribution unit and/or wiring that overheats and melts could create the risk of a fire. Corrective Actions: E-ONE will notify owners by mail and advise you to contact Weldon to update the Weldon power distribution unit software.

Under certain conditions, this could allow lowering of the aerial device when it should not be lowered. As a result, the truck could tip on its side. Safety Risk: A truck that tips over can create a risk of injury. Corrective Actions: E-ONE will notify owners by mail and provide you with instructions to either replace the sensors or provide a software update for the existing sensors.

    Issue: On certain fire trucks, engine exhaust could cause the truck body to overheat during periods of heavy use. If this happens, wiring and parts could deform, melt, or catch fire. Safety Risk: Overheated wiring or parts could create a fire risk. Corrective Actions: E-ONE will notify owners by mail and provide you with directions for adding an opening to the top of the body, and provide a protective grate to cover it.

    Issue: On certain fire trucks, a loose screw connection in the load center (circuit breaker panel) could cause it to overheat. Note: This recall is for fire trucks equipped with certain Square D QO plug-on neutral load centers. Safety Risk: An overheated load center could create the risk of a fire. Corrective Actions: E-ONE will notify owners by mail and instruct you to contact Schneider Electric to arrange for an inspection. The load center will be repaired or replaced, as necessary.

    Issue: On certain fire trucks, the fuel tubes between the fuel rail and the injectors may crack and create a fuel leak. Note: This recall only affects fire trucks equipped with a Cummins X12 diesel engine. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: Cummins will notify owners by mail and instruct you to take your fire truck to a Cummins authorized service center to replace the fuel tubes for cylinders 4, 5 and 6 and install vibration isolators.

    Issue: On certain vehicles, the fuel rail assembly could develop a leak. Note: This recall only affects vehicles equipped with a Cummins L9 or ISX12 diesel engine. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: Cummins will notify owners by mail and instruct you to take your vehicle to a Cummins authorized service center to inspect the fuel rail. If damage is found, the fuel rail and fuel lines will be replaced. If no damage is found, the fuel rail will be reassembled with new crush washers using a special tool.

    Issue: On certain vehicles equipped with a Weldon Vista IV control panel, an electrical problem could cause the screen to not work properly. As a result, the operator may not be able to control or access certain vehicle functions. Depending on the vehicle configuration, this can affect the emergency warning lights, patient care devices and/or cause the rearview camera image not to display. Safety Risk: Emergency personnel that do not have immediate access to vehicle controls could create a delay in emergency response. Additionally, a rearview camera image that does not display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: E-ONE will notify owners by mail and instruct you to take a photo of the Vista display screen(s) showing the decal with the serial number. E-ONE will verify if each unit is affected and provide a new display, as necessary.

    Issue: On certain vehicles, the starter lock-out relay could be damaged over time, causing it to fail. If this happens, it will not be possible to start the engine. Note: A possible symptom of a damaged starter lock-out relay is a truck that requires multiple attempts to start. Safety Risk: An emergency vehicle with an engine that cannot be started could increase the risk of injury in an emergency situation. Corrective Actions: E-One will notify owners by mail and instruct them to take their vehicle to a service center to install a relay and diode kit.

    Issue: On certain fire trucks, the front shock absorbers may separate from their upper mounts. A loose shock absorber may contact and damage the steering gear housing, resulting in a steering fluid leak and loss of power steering assist. Note: A loose shock absorber may cause a rattle or clunk noise from the front suspension or a change in vehicle handling. Safety Risk: A loss of power steering assist could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a service center to inspect and repair the front shock absorber upper mounts.

A failing relay could have visible cracks and may produce a burning smell. Safety Risk: An overheated relay could create the risk of a fire. Corrective Actions: The company will notify owners by mail. Owners will be provided with a new relay assembly and instructions on how to replace the existing one.

This could cause a spindle failure and result in the possible separation of a wheel from the vehicle, increasing the risk of a crash causing injury and/or damage to property.     Certain vehicles equipped with a Cummins ISL, L9, ISX15 or X15 engines, are fitted with a starter lock-out relay which could fail over time due to high voltage current from the starter solenoid during start up. In an emergency situation the vehicle would not be able to start, which could increase the risk of injury during firefighting operations. Correction: Dealers will inspect and replace the starter lock-out relay. Additionally a diode will be added to the starter solenoid circuit to improve longevity of the lock-out relay.

    On certain vehicles equipped with an IOTA 50 amp transfer switch which is used to switch power between on-vehicle generator and shoreline power. Under certain heavy duty cycles, the lugs on the buss bar may loosen. Under high electrical loads, loose electrical connections could cause the circuit to overheat, which would increase the risk of fire causing injury and/or damage to property. Correction: Dealers will replace the IOTA switch.

    On certain fire trucks, the retaining ring(s) in the relief valve assembly may not have been made using high grade stainless steel, causing it to deteriorate over time due to frequent exposure to water. If the retaining ring fails, the relief valve may break down and prevent the fire pump from building adequate pressure, which would increase the risk of injury during firefighting operations. Correction: Dealers or certified technicians will inspect and replace the retaining ring(s) as necessary.

    On certain vehicles, the steering assist cylinder attachment and adjustment points may not have been tightened to specification at time of assembly. This could result in the steering assist cylinder separating from the steering arm, causing a loss of steering assist and increasing the risk of a crash causing injury and/or damage to property. Correction: Vehicles will be inspected and fasteners tightened to specification.

    On certain vehicles equipped with Reyco independent front suspension (IFS), the suspension system may contain loose ball joints. If these are not detected through regular servicing, they may separate, causing suspension failure. This could affect vehicle handling and increase the risk of a crash causing injury and/or damage to property. Correction: Owners will be instructed the have the suspension inspected by an E-One certified dealer or technician. Loose ball joints will be replaced.

Suspension air bag failure could affect vehicle stability which could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will install a pressure protection valve on the exhaust port of the height control valve.

    On certain vehicles equipped with aerial devices, the hydraulic filter head for the hydraulic system may be defective. As a result, the filter head may flex under pressure, allowing the sealing O-ring to be pushed out, resulting in a fluid leak. This could cause reduced aerial/ladder performance, increased power requirements and could potentially result in damage to other hydraulically driven components. This could increase the risk of injury/damage to property. Correction: Dealers will inspect the filter head and replace if necessary.

    On certain fire trucks, the components that make up the 6" water intake strainer may have been improperly soldered during the manufacturing process. This could lead to degradation in pump performance which could reduce firefighting effectiveness, and potentially increase the risk of injury. Correction: Hale Products recommends an inspection be performed by pulling caps and any suction adapters off of the intake and check the strainer for faulty soldering. If the strainer is found to be missing solder at the joint, Hale Products will send a replacement strainer directly to affected owners.

If the vehicle Master Power Switch is shut off while the brake pedal is depressed and the switch is turned back on, the brake lights could remain illuminated when the brakes are not applied. Erratic brake lamp function could increase the risk of a crash causing injury and/or property damage. Correction: Dealers will update software.

Parking brake drag could result in a fire causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the park brake actuators.

    On certain fire trucks, specific operating conditions (such as tight, successive, highly banked curves in opposite directions), could trigger unintended Electronic Stability Control (ESC) system intervention. As a result, the ESC may unnecessarily apply one of the front brakes in order to correct the perceived oversteer condition. This may cause the vehicle to deviate from the intended path, thereby increasing the risk of a crash causing property damage and/or personal injury. Correction: Dealers will replace the ESC module.

  • Accessories#2011336

    On certain fire trucks, the door latch fasteners may not have received adequate tightening torque during the manufacturing process. As a result, the outer door handle may rotate out of position, allowing a retention spring to dislocate, which could cause the door to open during a vehicle crash. Should this occur, unbelted occupants could fall out of the vehicle and sustain personal injury. Correction: E-One service centers will replace the door latch assemblies.
