    On certain vehicles, the castellated nut and cotter pin on the steer axle may not have been tightened to specification. This could cause the tie rod to become loose in the steer axle, which would result in audible noise and/or looseness in steering. If the vehicle were continued to be operated in this condition, the tie rod could disconnect from the knuckle, resulting in a complete loss of control of a vehicle wheel, which would increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will inspect the torque of the castellated nuts on the tie rod, and replace the tie rod stud and knuckle as necessary.

    On certain vehicles, the front hubs do not support the gross axle weight rating of the front axle. Specifically, 5,987 Kg (13,200 lbs.) rated front hubs were used on trucks requiring a 6,622 Kg (14,600 lbs.) rated front axle. If the axle load exceeds the maximum weight capacity of the hub, there is an increased risk of hub and bearing failure, which could cause a complete separation of the wheel(s) and a loss of vehicle control, increasing the risk of a crash causing injury and/or damage to property. Correction: Dealers will replace the front hub assemblies with hub assemblies rated for 6,622 Kg (14,600 lbs). Note: If damage occurs due to overloading, the ABS Malfunction light may illuminate as the distance between the ABS tone ring and the sensor increases (i.e. sensor loses contact with the tone ring).

    On certain vehicles, the cap nut that retains the interaxle driveshaft yoke to the rear axle input shaft may prematurely loosen. If the nut were to loosen and come off, the yoke could separate from the axle input shaft and cause the drive shaft to disconnect. A disconnected drive shaft would result in a loss of motive power, and create a road hazard, both of which could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will inspect the torque on the cap nut, and the nut will be replaced if the torque is found under specification. Note: This is an expansion of recall 2016-079.

    On certain vehicles outfitted with Goodyear G399A LHS 295/75R22.5 LRG tires, the tires may not have been cured properly during manufacturing. This could cause the tires to have incomplete adhesion, which could lead to partial tread separation and/or tire failure. If the tread were to separate while a vehicle is in motion, it could cause a loss of vehicle control, which would increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will replace affected tires with similar tires.

    On certain vehicles, the front axle beam may have been improperly heat treated, affecting material properties. This could lead to fracture of the axle beam at the kingpin, potentially causing lead to a loss of vehicle control and a vehicle crash causing injury and/or damage to property. Correction: Dealers will inspect the axle beam for batch code and serial number markings and replace affected axles. Note: There may be evidence of the beam deforming before the knuckle and king pin loosens from the beam.

    On certain vehicles, the fifth wheel locking mechanisms could be susceptible to damage and/or wear. This could cause the locking mechanism to fail properly engage, potentially resulting in a towed trailer unit separating from the truck tractor. As a result, the trailer could strike another vehicle, stationary object, or bystander, causing injury and/or damage to property. Correction: Fontaine dealers will replace all Ultra LT fifth wheel units with Ultra NT model fifth wheel units.

This could cause the electrical cables and air lines to fall down and get twisted in the driveline. This could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will inspect and repair the vehicle as needed.

    On certain vehicles, the fifth wheel remote unlocking switch may not work as designed. As a result, the driver may inadvertently release the 5th wheel latch when the park brake is applied if he/she presses or bumps the switch. This could cause the trailer to decouple from the truck increasing the risk of injury and/or damage to property. Correction: Dealers will add a redundant measure such as an additional switch or adding a thumb lock that requires 2 actions to push the switch.

    Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard 121 - Air Brake Systems. These vehicles were built with insufficient air tank reserve capacity, which may provide an insufficient air supply to properly operate the brakes during repetitive brake applications. This may result in reduced braking performance which could result in a vehicle crash. Correction: Dealers will install a bigger air tank to increase air reservoir capacity.

    On certain model vehicles outfitted with an Automatic Over-Speed Protection system, the system may be incorrectly wired. This presents a potential risk as the operator may not be able to control an unintended increase in the engine RPM if combustible vapors enter through the air intake. If this occurs, it could increase the risk of an engine fire that may cause injury and/or property damage. Correction: Dealers will inspect and repair accordingly.

    On certain model vehicles, water around the windshield can migrate into the Electronic Power Distribution Module and contaminate it. As a result, circuits may be bridged creating an electrical short. This could potentially result in a fire, increasing the risk of injury and/or damage to property. Correction: Dealers will inspect the module, and if found contaminated, will replace it. Additional measures will be taken to address the leak if contamination is found. Note: This is an expansion of recall 2014-053.

    On certain vehicles, the collars that prevent the ball sockets from backing out of the tie rod may have been incorrectly positioned. If the ball socket(s) became loose, a complete loss of steering or loss of steering at one wheel end could occur, which could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will inspect, and if necessary, reposition the collar.

    On certain vehicles, bolts securing the seat belts to the seat frame, cab wall and cab floor may have been insufficiently tightened during vehicle assembly. As a result, bolts could loosen over time and potentially allow the seat belt to detach from its mounting points. If this occurs, the seat belt may fail to restrain the seat occupant in a crash, increasing the risk of injury and/or damage to property. Correction: Dealers will tighten the affected bolts to the specified torque.

    On certain vehicles, the air brake supply tank pressure release valve may not have been installed at time of assembly. This could allow brake system pressure to rise above allowable limits, potentially causing the air tank to rupture. This could cause a loss of air brake system pressure, resulting in an eventual loss of service brake function and an unintended application of the emergency brakes, and could pose a risk of injury to bystanders due to flying debris. Correction: Dealers will install a pressure release valve in the supply tank.

Conspicuity tape applied to the back of the cab may be obscured by exhaust system components to a greater degree than what is allowed by the standard. This could reduce vehicle conspicuity during hours of darkness, potentially increasing the risk of a crash causing injury and/or damage to property. Correction: Dealers will add reflectors if necessary.

Actuation of the turn signal or hazard lights in this condition would cause them to remain illuminated and not flash as intended. As a result, drivers may not be able to clearly signal their intentions to turn, change lanes or warn others in an emergency situation. Correction: Dealers will replace the turn signal/hazard flasher module.

    On certain vehicles equipped with Cummins compressed natural gas engines, incorrect engine ECU programming could cause the exhaust system temperature to become elevated during cold weather operation. This could also cause flames to be emitted from the exhaust. As a result, nearby combustible material could be ignited, increasing the risk of a fire, injury and/or damage to property. Correction: Authorized Cummins service facilities will update software programming.

Correction: Dealers will inspect and repair vehicles as required.

Correction: Dealers will inspect repair vehicles as required.

    On certain trucks, the cotter pin which retains the brake slack adjuster’s clevis pin may have been omitted during vehicle assembly. As a result, the clevis pin could loosen and fall out, causing a loss of brake function at the affected wheel. This could cause the vehicle to pull to one side under braking, or increase stopping distance, which could result in a crash causing property damage and/or personal injury. Correction: Dealers will verify cotter pin installation on each brake slack adjuster. Replacement cotter pins and/or clevis pins will be installed as needed.

    On certain vehicles, the wheel rims may have been manufactured incorrectly and could fail. A wheel separating from the vehicle could cause a loss of control and a crash. In addition, the errant wheel assembly could strike a vehicle, stationary object, or a bystander. These issues could result in property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the wheel rims. Note: Further evaluation revealed that no Canadian vehicles are involved.

    On certain trucks, an electrical short circuit in the fog light circuitry could result in a loss of low beam headlamp function. Loss of headlamp illumination, should it occur during a rainy/snowy day, or during hours of darkness, could compromise the driver’s ability to see the road and its users, which could result in a crash causing property damage and/or personal injury. Correction: Dealers will add an extra wiring harness between the cab harness and the Daytime Running Lamp (DRL) module, in order to isolate the fog light circuit.

    On certain vehicles, the hardware used to affix the windshield wiper linkage to the wiper motor may have been subject to rust and corrosion before assembly. This contamination may have prevented the bolt/nut from being tightened adequately. As a result, the bolt/nut may loosen to a point where the linkage detaches from the wiper motor, which would cause the wipers to stop functioning. Loss of windshield wiping capability, should it occur during a rainy/snowy day, may compromise the driver's ability to see the road and its users, which could result in a crash causing property damage and/or personal injury. Correction: Dealers will replace the hardware in question.

    On certain trucks equipped with Bendix ATR-6 Antilock Traction Relay valves, in extremely cold conditions (at or below -18 degrees Celsius), internal ATR valve leakage can occur. This could cause unintended brake application, which could overheat the affected brakes and cause a fire. Also, in certain low traction conditions, unexpected brake application can cause a loss of vehicle control and a crash. These issues could result in property damage and/or personal injury. Correction: Dealers will update the cover assembly on the ATR valves.

    On certain trucks, the clutch pedal assembly may have been manufactured incorrectly and could fail. This could allow the clutch pedal to fall to the floor, prohibiting clutch operation, and causing a reduction in braking efficiency. Increased stopping distances could result in a crash causing property damage and/or personal injury. Correction: Dealers will replace the clutch pedal assembly.

    On certain trucks, the driveshaft may not have been welded correctly during the manufacturing process. This could allow the driveshaft to fracture along the length of the tube seam weld when torque is applied. The fractured tube can twist or sever, which could result in additional damage to the driveline, the vehicle to which it is attached, and any additional nearby components. Should the brake air lines become damaged, it could allow spring brake application, causing immediate brake lock-up. If this occurs while the vehicle is in motion, it could result in a crash. Also, failure of the driveshaft would result in lost vehicle propulsion which, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash. Either condition could result in property damage and/or personal injury. Correction: Dealers will replace the driveshaft assembly.

    On certain vehicles, the slack adjuster housings may have been manufactured incorrectly, which could allow the housings to fracture and fail. As a result, the wheel brake to which the slack adjuster is attached would no longer function, which could increase vehicle stopping distances. Should a steer axle slack adjuster fail, it could also cause the vehicle to pull to one side during braking. Either problem could result in a crash causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the slack adjusters.

    On certain trucks, the bolts used to secure the fifth wheel hitch assembly to the frame may not have received adequate tightening torque during vehicle assembly. This could allow the mounting bolts to fail and the fifth wheel assembly, as well as the trailer, to separate from the tractor. A towed unit separating from the towing vehicle could strike another vehicle, stationary object, or bystander, causing property damage and/or personal injury. Correction: Dealers will inspect the mounting bolts and replace as required.

    On certain trucks, the rear hold-down brackets that secure the battery box lid may be rotated too far forward, which may restrict the battery box lid from properly locking into place. If this occurs, the battery box lid may come off when stepping on the step, which may cause a person to fall from the vehicle resulting in personal injury. Correction: Dealers will inspect and adjust the rear hold-down brackets as required.

    On certain vehicles, the rear conspicuity reflector typically found on the mud flap hanger brackets may not have been installed in the back of the cab which is the alternate position allowed for vehicle manufactured without mud flap hanger brackets. Missing reflector is a non-compliance with Canada Motor Vehicle Safety Standards 108 - Ligthing System and Retroreflective Devices. Correction: Dealers will inspect and if missing will install the required reflectors.
