    Issue: Under certain conditions after the fire truck has been auto-leveled, a software problem could cause the firm-on-ground system light to remain on after the rear stabilizer loses contact with the ground. As a result, the fire truck could shift when the aerial device is being used. Note: This recall only affects certain fire trucks equipped with a single rear axle. Safety Risk: A fire truck that shifts while the aerial device is being used could create a risk of injury. Corrective Actions: Pierce will notify owners by mail and provide you with instructions for how to update the vehicle software and perform an aerial device functionality check. Alternatively, you may schedule an appointment to have the software update performed at a Pierce dealership.

    Issue: On certain fire trucks, the fuel tubes between the fuel rail and the injectors may crack and create a fuel leak. Note: This recall only affects fire trucks equipped with a Cummins X12 diesel engine. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: Cummins will notify owners by mail and instruct you to take your fire truck to a Cummins authorized service center to replace the fuel tubes for cylinders 4, 5 and 6 and install vibration isolators.

    Issue: On certain vehicles, the fuel rail assembly could develop a leak. Note: This recall only affects vehicles equipped with a Cummins L9 or ISX12 diesel engine. Safety Risk: A fuel leak can create the risk of a fire. Corrective Actions: Cummins will notify owners by mail and instruct you to take your vehicle to a Cummins authorized service center to inspect the fuel rail. If damage is found, the fuel rail and fuel lines will be replaced. If no damage is found, the fuel rail will be reassembled with new crush washers.

    Certain vehicles may have driveshaft universal joint bearing cap fasteners that were not properly tightened at the time of assembly. This could allow bearing caps to detach from the vehicle and become a road hazard. Additionally, this could cause the universal joint to separate causing a loss of motive power which could increase the risk of a crash causing injury and/or damage to property. Correction : Dealers will inspect and retighten the fasteners as necessary.

    On certain vehicles, the lower control arms may have developed cracks at time of manufacture and may fail. This could cause partial suspension failure and could result in a loss of control, increasing the risk of a crash causing injury and/or property damage. Correction: Owners will be provided with inspection instructions to determine if the control arms may be subject to the recall. Those found to be potentially affected will be inspected by the manufacturer and replaced as necessary.

    On certain trucks, the trailer wiring connector may have been incorrectly wired so that a stop signal overrides the turn signal. As a result, trailer lights would not indicate a turn while braking. This could increase the risk of a crash causing injury and/or damage to property. Correction: Dealers will install a replacement wiring harness.

    Certain fire trucks fail to conform to Canada Motor Vehicle Safety Standard 206 – Door Lock and Door Retention Components. Specifically, the outer door handle spring is not sufficiently stiff, which may allow the door to open during a vehicle crash. As a result, an unbelted occupant could fall from the vehicle and sustain personal injury. Correction: Dealers will replace the outer door handle actuator spring with an updated version.

    On certain fire trucks, the inner door handles may have been manufactured incorrectly, which could allow the door to open unexpectedly during vehicle operation. Should a door open while the vehicle is underway, an unbelted occupant could fall from the vehicle and sustain personal injury. Correction: Dealers will replace the inner door handle and door linkage rod on all cabin doors.

    On certain fire trucks equipped with a Detroit Diesel DD13 engine, the support system for the high pressure fuel line (between the fuel pump and the fuel delivery rail) may be damaged during a service repair procedure. Incorrect torque applied to the dampener "P" clip fasteners may damage the fasteners, which could allow the clips to loosen. This could result in a cracked fuel line and a diesel fuel leak. Diesel fuel leakage may ignite on a hot engine surface and cause a fire. As well, diesel fuel spilt on the roadway could endanger other road users by making the roadway unexpectedly slippery. These issues could result in property damage and/or personal injury. Correction: Dealers will replace the high pressure fuel line and install a more robust fuel line support system.

    On certain emergency vehicles, the engine shutdown system (normally provided to protect the engine in case of malfunction) may not have been disabled during vehicle assembly. Unexpected emergency vehicle engine shutdown, under certain conditions, could hamper rescue operations and place the public at risk. Correction: Authorized service facilities will disable the engine shutdown system.

    On certain fire trucks, specific operating conditions (such as tight, successive, highly banked curves in opposite directions), could trigger unintended Electronic Stability Control (ESC) system intervention. As a result, the ESC may unnecessarily apply one of the front brakes in order to correct the perceived oversteer condition. This may cause the vehicle to deviate from the intended path, thereby increasing the risk of a crash causing property damage and/or personal injury. Correction: Dealers will replace the ESC module.

  • Engine#2011299

    On certain fire trucks equipped with a Cummins engine, the V-band clamps used to connect the inlet and outlet cone sections to the Diesel Oxidation Catalyst (DOC) / Diesel Particulate Filter (DPF) include a T-bolt which can fracture and fail when subjected to stress or load. A fracture or failure of the T-bolt may cause the clamp to loosen and the inlet or outlet sections attached to the DOC/DPF housing to disconnect. Should this occur, hot exhaust gases can vent prior to exiting the tailpipe, creating a risk of combustion or damage to adjacent materials, which could result in a vehicle fire. If both clamps fail, the DOC/DPF housing could detach completely, separate from the vehicle, and strike another vehicle, stationary object, or bystander. These issues could result in property damage and/or personal injury. Correction: Cummins will install updated V-band clamps.
