    Issue: On certain vehicles, if the front bearing on the alternator fails, the alternator belts could continue to turn the alternator. This could cause the alternator bearing to overheat. Safety Risk: An overheated alternator bearing could create the risk of a fire. Corrective Actions: Prevost will notify owners by mail and instruct you to take your vehicle to a Prevost service center to replace the alternator belts. To reduce the safety risk, Prevost recommends that you should inspect the front bearing on the alternator regularly, and not operate the engine with a defective alternator.

    On certain buses equipped with Kidde fire extinguishers with black plastic handles and valves, the fire extinguishers may become clogged or require excessive force to activate. Also, the nozzle may detach from the valve assembly with enough force that it could cause injury, and fail to discharge. In case of fire, failure of the extinguisher to function could result in injury and/or property damage. Correction: Owners will be provided with instructions on how to inspect the fire extinguisher to see if it is recalled. Replacement fire extinguishers will be supplied by Kidde directly.     On certain vehicles, the fuel tank cradle bolts could loosen and fail.  If this occurs, the fuel cradle and fuel tank could drop to the ground, which could disable the vehicle, increasing the risk of a crash.  This could also potentially result in a fuel leak.  Fuel leakage, in the presence of an ignition source, could result in a fire causing property damage and/or personal injury.  Correction:  Dealers will repair and shim the attachment points, and the hardware will be replaced.

    On certain buses outfitted with Automatic Fire Suppression System (AFSS) Linear Thermal Detectors (LTD), the routing of the wiring for these systems could be compromised by normal engine compartment temperatures. If this were to occur, it could result in the AFSS system activating and giving a false warning to the driver, potentially resulting in a stopped vehicle, increasing the risk of a vehicle crash. Correction: Dealers will inspect, and reroute the wiring.

    On certain buses, a fuel line retaining clip may be missing, which can result in the fuel line rubbing against a braided coolant hose. If there was contact, the fuel line may be compromised over time, which can result in spraying fuel onto hot exhaust components potentially resulting in an engine fire causing injury and/or damage to property. Correction: Dealers will inspect and replace the fuel line, as necessary, and ensure that the fuel and coolant lines are properly secured.

    On certain vehicles, the accelerator pedal return springs may break, causing the engine to fail to return to idle. This could increase the risk of a crash causing injury and/or damage to property. Correction: dealers will inspect the pedal assembly and replace if necessary.

    On certain vehicles, the Vehicle Certification Label contains incorrect front and rear tire pressure rating. The correct tire pressure rating is 120 psi. Incorrect tire pressure information may lead to overinflation of tires and could affect vehicle stability, braking and traction, potentially resulting in a loss of control or increased braking distances. This could result in a crash causing injury and/or property damage. Correction: Dealers will replace affected labels.

    On certain vehicles, the drag link adjustment collar bolt could contact the pitman arm. As such, the drag link turnbuckle could fracture and fail. A sudden loss of steering control could result in a crash causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the drag link turnbuckle assembly.

    On certain vehicles, the engine calibration for the “limp home” mode was inadvertently set too high at 1750 rpm instead of 1200 rpm. Should the “limp home” mode be activated, the increased idle speed may cause sudden and unexpected acceleration, which could result in very high vehicle speeds and make it difficult to slow or stop the vehicle if the transmission is not shifted into neutral. This could result in a crash causing property damage and/or personal injury. Correction: Dealers will install updated engine calibration software.

    On certain vehicles, the thermal detector could become damaged, causing inadvertent activation of the Automatic Fire Detection and Suppression System (AFSS), which would result in engine shut down. Loss of vehicle propulsion, in conjunction with traffic and road condition, and the driver’s reactions, could increase the risk of a crash causing property damage and/or personal injury. Correction: Dealers will insulate the AFSS thermal detector.

    On certain vehicles, a component of the Automatic Fire Detection and Suppression System may have been installed incorrectly and could contact or rub against brackets and clamps. As a result, the system may inadvertently activate causing, amongst other things, the engine to stop. Engine stalling would result in lost vehicle propulsion which, in conjunction with traffic and road condition, and the driver's reactions, could increase the risk of a crash causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, replace the linear thermal detector.

    On certain vehicles, the accelerator pedal throttle position sensor could fail. A faulty throttle position sensor will send a permanent idle signal to the Engine Control Unit (ECU). As such, depressing the accelerator pedal will provide no throttle response whatsoever. Loss of vehicle propulsion, in conjunction with traffic and road conditions, and the driver’s reactions, could increase the risk of a crash causing property damage and/or personal injury. Correction: Dealers will replace the accelerator pedal throttle position sensor.

    On certain vehicles, the fuel support assembly may have been incorrectly welded during the manufacturing process. Should certain welds fail, portions of the support structure could contact the ground or separate from the vehicle. Sections of the fuel tank support separating from the vehicle could strike another vehicle, stationary object, or bystander, causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, reinforce the fuel support assembly.

    On certain vehicles equipped with a Ricon F9 series cassette wheelchair lift, an improperly manufactured switch in the wheelchair lift restraint buckle may allow operation of the lift with the safety belt not fully latched. If the belt is not properly fastened, a wheelchair may not be adequately protected against unwanted movement, resulting a fall risk, potentially causing serious injury or death. Correction: The manufacturer will provide instructional information and warning decals.

    Certain aftermarket replacement steering drag link tie rod ends may have been manufactured incorrectly. In certain heavy loading conditions the tie rod end could break, which may result in a loss of steering control and a crash causing property damage, personal injury or death. Correction: Owners who had drag link tie rod ends replaced using these faulty components are asked to take their vehicle to a Prevost dealer for inspection and repair if required.
