Safety Risk: A child seat that is not properly installed could create an increased risk of injury in a crash. Corrective Actions: The company will notify owners by mail. Owners will be sent the missing anchorage parts and instructions for their installation.

Issue: On certain school buses, the restraining barriers may deform in a crash. This problem only affects 39 inch (99 cm) restraining barriers built by HSM, with model number FB11-39. Safety Risk: A restraining barrier that fails in a crash could increase the risk of injury to occupants. Corrective Actions: The company will notify owners by mail. Restraining barriers will be made stronger with a reinforcement bracket. Owners will be provided with instructions for installing the bracket.

Correction: Dealers will replace the seat bottom assembly.

Certain school buses fail to conform to the requirements of Canada Motor Vehicle Safety Standard 217 – Bus Window Retention, Release and Emergency Exits. The split sash emergency egress windows may have an improperly installed retainer bar, insufficient strength of the window latch connecting rod, and/or missing or failed window fasteners. In the event of an emergency, passengers may not be able to quickly exit the bus, which could increase the risk of personal injuries. Also, if all the fasteners on a single retaining bar failed, the window could separate from the vehicle and strike another vehicle, a stationary object, or a bystander, causing property damage and/or personal injury. Correction: Dealers will inspect and, if necessary, modify window installation.

If this happens and goes unnoticed by the driver before he/she pulls away, the passenger could be dragged by the bus, potentially causing serious personal injury. Correction: Dealers will inspect and, if necessary, replace the door bracket with an updated version, and install a shield where the support hardware is attached to the door post.
