Issue: On certain aerial devices, the bolts for the pedestal and turntable may fail. If multiple bolts fail, the turntable and pedestal could separate from the vehicle. Safety Risk: If the pedestal and turntable separate from the vehicle, there may be a risk of injury. Corrective Actions: The company will notify owners by mail and advise them to perform a visual inspection of the rotation bearing bolts. If any bolts are marked with the number "6043”, the owner must stop using the aerial device. The bolts must be replaced before returning the unit to service.
