    Issue: On certain aerial devices, the bolts for the pedestal and turntable may fail. If multiple bolts fail, the turntable and pedestal could separate from the vehicle. Safety Risk: If the pedestal and turntable separate from the vehicle, there may be a risk of injury. Corrective Actions: The company will notify owners by mail and advise them to perform a visual inspection of the rotation bearing bolts. If any bolts are marked with the number "6035”, the owner must do a torque check. If any of the bolts break during the torque check, take the unit out of service and replace the bolts. The bolts marked with the number 6035 that pass the torque check will also have to be replaced within 30 days.

Safety Risk: If the pedestal and turntable separate from the vehicle, there may be a risk of injury. Corrective Actions: The company will notify owners by mail and advise them to perform a visual inspection of the rotation bearing bolts. If any bolts are marked with the number "6043”, the owner must stop using the aerial device. The bolts must be replaced before returning the unit to service.
