Issue: On one digger derrick, there is no cover on the controls for opening and closing the pole guide. As a result, an operator may unintentionally operate the open/close controls while performing another boom function. If an unbalanced pole is being manipulated, it could be released, resulting in loss of control of the load. Safety Risk: If an operator loses of control of the load, it could create a risk of injury. Corrective Actions: Altec Industries will notify the owner by mail and provide them a guard kit to install it over the pole guide controls.
