Issue: On certain digger derricks, the welds for the pedestal tube could crack. If the cracks progress, they could result in a failure of the pedestal and uncontrolled movement. Safety Risk: If the pedestal were to fail, the risk of injury and/or property damage would be increased. Corrective Actions: The company will notify owners by mail. Owners will be reminded to perform a daily inspection of the pedestal, as in the owner’s manual. They will also be provided with a reinforcement kit and directions for how to inspect the pedestal for cracks. If cracks are present, the unit must be taken out of service and repaired.

On certain truck-mounted digger derricks, the bolts which secure the winch motor may not have been adequately tightened during assembly. If the bolts loosen, the motor could detach, allowing a load carried by the winch to fall, placing anyone near or beneath at risk of personal injury. Correction: Altec will direct vehicle owners to inspect and tighten the winch motor mounting bolts.

On certain truck-mounted digger derricks, the drum for the boom tip winch may deform or fracture, cutting the synthetic winch line. This could allow a load carried by the winch to fall, placing anyone near or beneath at risk of personal injury. Correction: Altec will direct vehicles owners to inspect and, if necessary, replace the winch drum.
