Issue: On certain digger derricks, the welds for the pedestal tube could crack. If the cracks progress, they could result in a failure of the pedestal and uncontrolled movement. Safety Risk: If the pedestal were to fail, the risk of injury and/or property damage would be increased. Corrective Actions: The company will notify owners by mail. Owners will be reminded to perform a daily inspection of the pedestal, as in the owner’s manual. They will also be provided with a reinforcement kit and directions for how to inspect the pedestal for cracks. If cracks are present, the unit must be taken out of service and repaired.

If the bolts loosen, the motor could detach, allowing a load carried by the winch to fall, placing anyone near or beneath at risk of personal injury. Correction: Altec will direct vehicle owners to inspect and tighten the winch motor mounting bolts.
